Antidote: 

Naloxone is used to reverse respiratory depression. Due to Isofentanyl’s potency, multiple doses of naloxone may be necessary.

Pharmacokinetics and Dynamics

Metabolism: 

Isofentanyl is metabolized in the liver, primarily via the cytochrome P450 enzyme system (CYP3A4). Its metabolites are excreted through the urine.

Drug-Drug Interactions: 

Isofentanyl has dangerous interactions with other CNS depressants, such as alcohol, benzodiazepines, or barbiturates, which significantly increase the risk of respiratory depression and fatal overdose. Drugs that inhibit or induce liver enzymes (e.g., CYP3A4 inhibitors) may alter its metabolism, leading to increased potency or reduced clearance.
