A synthetic opioid classified as a benzimidazole derivative. It is an ultra-potent opioid analgesic with potency estimated to be 100–1000 times stronger than morphine.

Etonitazene is vastly more potent than most opioids, with an efficacy similar to or exceeding that of other benzimidazole opioids like isotonitazene. Even microgram doses can cause profound opioid effects, making it highly dangerous in overdose.

It is abused recreationally for its powerful euphoric and sedative effects. Due to its potency, it is often found in illicit markets in small quantities or as an adulterant in counterfeit pills or powders.
