Treatment Options
Supportive Care:
Artificial respiration, oxygen therapy, and intensive care monitoring. Mechanical ventilation may be required in severe cases to sustain breathing until the opioid is cleared from the body.
Antidote:
Immediate administration of naloxone (Narcan), but due to the drug’s extreme potency, multiple high doses are often needed.
Pharmacokinetics and Dynamics
Metabolism:
Ohmefentanyl is metabolized in the liver, primarily through the CYP3A4 enzyme system, similar to fentanyl. It is broken down into inactive metabolites and excreted mainly through urine. Because of its high potency, its effects may last longer than fentanyl, depending on the specific isomer and route of administration.
Drug-Drug Interactions:
Ohmefentanyl has dangerous interactions with other central nervous system depressants, including benzodiazepines (e.g., alprazolam, diazepam), alcohol, barbiturates, and other opioids. These combinations significantly increase the risk of fatal respiratory depression. Additionally, CYP3A4 inhibitors (such as grapefruit juice, certain antibiotics, or antifungals) can prolong its effects and increase overdose risk.
