Supportive Care:
Supportive care strategies, similar to amphetamine, cocaine (9). Benzodiazepines for severe agitation, sedation if needed for psychosis, IV fluids for dehydration, cooling measures for hyperthermia, and cardiac monitoring in cases of suspected heart involvement.
Antidote:
There is no specific antidote for α-PiHP. Treatment focuses on symptom management, typically with benzodiazepines and aggressive supportive care.
Pharmacokinetics and Dynamics
Metabolism:
Metabolism is expected to be similar to α-PHP (35). α-PiHP is believed to be metabolized in the liver via cytochrome P450 enzymes, producing several metabolites that are excreted by the kidneys. Standard drug screens usually do not detect α-PiHP unless specialized synthetic cathinone panels are used.
Drug-Drug Interactions:
Combining α-PiHP with other stimulants (like methamphetamine or cocaine) greatly increases the risk of heart attacks, strokes, and overheating. Concurrent use with depressants like alcohol or benzodiazepines can lead to unpredictable sedation and cardiovascular strain, sometimes masking overdose warning signs.
