Acute benzodiazepine intoxication is primarily managed with supportive care, which may include airway protection and, in severe cases, endotracheal intubation to ensure adequate ventilation.
Antidote:
While the antidote flumazenil can effectively reverse benzodiazepine-mediated sedation, its use in acute benzodiazepine intoxication is not recommended due to the potential risks that may outweigh benefits as it can precipitate acute withdrawal syndromes, lower seizure threshold, and cause cardiac dysrhythmias.
Pharmacokinetics and Dynamics
Metabolism:
Nimetazepam is metabolized in the liver, primarily into nitrazepam and 7-aminonimetazepam before being excreted in urine. It has a long half-life (10-24 hours), meaning its effects can last well into the next day, increasing the risk of next-day sedation.
Drug-Drug Interactions:
Nimetazepam potentiates the effects of alcohol, opioids, and other depressants, significantly increasing the risk of fatal respiratory depression. Combining it with stimulants (like amphetamines) can lead to unpredictable effects, including cardiovascular stress.
