Tetrahydrofuranylfentanyl
Found in:
Fentanyl
Type/Reason added:
Fentanyl Analog
Relative Potency:
20% of fentanyl
Treatment Options
Supportive Care:
Oxygen supplementation and ventilation support is critical in overdose cases.
Antidote:
Treatment of Tetrahydrofuranylfentanyl overdose requires rapid administration of naloxone (Narcan), an opioid antagonist. High or repeated doses of naloxone may be needed due to the drug's potency and duration.
Pharmacokinetics and Dynamics
Metabolism:
Tetrahydrofuranylfentanyl is metabolized primarily in the liver, likely through cytochrome P450 enzymes, particularly CYP3A4, similar to fentanyl. Its metabolites are excreted mainly through the urine.
Drug-Drug Interactions:
Tetrahydrofuranylfentanyl has dangerous interactions with other central nervous system depressants, including benzodiazepines, alcohol, barbiturates, and other opioids. These combinations greatly increase the risk of fatal respiratory depression. CYP3A4 inhibitors (like certain antibiotics and antifungals) may slow metabolism and heighten its effects.
