Nitazenes belong to the 2-benzylbenzimidazole class of opioids and act as potent synthetic opioid agonists at the mu-opioid receptor. First developed in the 1950s as potential analgesics, they were never approved for medical use due to their extreme potency and overdose risk.

Nitazenes can be significantly more potent than fentanyl, with some analogs (e.g., isotonitazene, etonitazene) being 20–100 times stronger than fentanyl. Their potency varies depending on the specific compound, but even trace amounts can be lethal.
